RESUMO

Diagnosis of alpha1-antitrypsin deficiency, a rare genetic disorder, consists of biochemical serum analysis to evaluate protein deficiency and electrophoretic abnormalities, as well as genetic analysis to identify SERPINA1 gene variants responsible for the protein deficiency. In majority of European countries diagnostic is centralized, blood and/or DNA samples are referred to dedicated reference laboratories. We aimed at analyzing data reported by the 8 leading European centers (AATD LABnet part of the EARCO ERS CRC and ERN-LUNG activities) to assess the effect of COVID-19 pandemic and healthcare disruption on the AATD diagnostic provision. In total, the number of tests dropped in 2020 vs 2019 by -20,43% (+/-13,25) for AAT concentration analysis, -25,5% (+/-12,79) for AAT phenotyping and -23,57% (+/-10,28) for SERPINA1 genotyping, while in 2021 vs 2019 by respectively -16,86% (+/-21,34), -9,2% (+/-30,38), -13,57% (+/-40,82). Specifically, diagnostics in Poland and Italy were affected the most with -48% and -28% fewer samples analyzed (2021 vs 2019, phenotyping), while in France and Portugal referrals recovered by the end of 2021, with + 18% and +29% respectively (2021 vs 2019, genotyping). Disorganization of healthcare provision due to the COVID-19 pandemic severely disrupted AATD diagnostics in Europe. In some countries the detrimental effect was temporary, while in others it continued in 2021.

RESUMO

The article presents a reflection regarding the linearity of causal relationships that involve environmental changes and their impact on (human) subjectivity. The environmental changes are here identified as transformations of natural, climatic, and socio-cultural systems resulting from contemporary lifestyle, and the impacts as the dimension of feelings and sensations related to these changes. The authors carried out conceptual theoretical debates to situate the covid-19 pandemic as a disaster associated with the contemporary way of life present in the urbanization process of cities. In moments of intense environmental change, such as the dystopian scenario represented by the advent of COVID-19, the more daily life is reconstituted at the confluence between the local and the global, the more individuals suffer, develop fears, and are challenged to develop survival strategies. Thus, subjectivity is more than a consequence of the pandemic disaster, it is conceptualized as a transformative potential - a dynamic continuum - expanding its scope as a perspective for action facing disaster. Future uncertainties may trigger processes to constitute new realities through political opportunities that favor creativity, seen as a coping strategy. The resumption of adaptive paths in the face of ongoing transformations may be possible, whether the "poetics of existence" finds its emergency and performance space.

RESUMO

This paper reports on a research (FAPESP 2019/08044-3) on the importance of sustainability, through the 17 Sustainable Developments Goals (SDGs) defined by Agenda 2030 with a particular emphasis to SDG 5 which deals with Gender Equity, and the importance of preventing and combating violence against women and girls. Since WHO’s declared the COVID 19 pandemic in March 2020, there has been a significant increase in domestic violence rates. Social isolation and hygiene practices are the most effective measures to prevent the spread and containment of the virus. Several feminist, national and international NGOs and societies have warned through their publications and social networks about these significant increases in this period. This chapter addresses this fact, through a systematic survey of these publications (e.g. UN Women, Patrícia Galvão Institute) and papers published in some relevant Journals (e.g. The Lancet). This reflection aims not only to contribute to the ongoing research, but also to present strategies related to SDG 5, and to offer proposals and approaches to face the challenges related to Gender Equity. © The Author(s), under exclusive license to Springer Nature Switzerland AG 2021.
